Check your email to get your Patient Portal Access (may take up to 24 hours).
STEP 4
Complete the Patient Portal
Log into the Patient Portal and complete the following sections:
- My Account → Personal Information
- My Account → Additional Information
- Questionnaires → Patient Forms → Insurance Info
STEP 5
Send us a message
Message us using the Patient Portal to request for an appointment:
- Messages → Ask Practice
- Type “New Patient Appointment Request” in the Subject field
- Type your date and time preferences on the Messages field
- Click Submit
